Or just embroider the name/wishes directly onto the chalk cloth. You can even use common cotton fabrics, felt etc to make these gift tags.
Designs as pictured. The designs fits in your 4x4 inch hoop and the back of the tag is nicely covered with fabric as well. All is done in-the-hoop on your embroidery machine.
